If you would prefer not to pay online using PayPal or your credit card, please complete the following order form, which will allow you to mail in your order, and pay by check or money order.
Upon receipt of your order, our Sales department will be in contact with you to confirm your order.
- Download the appropriate Excel order form (California residents, or non-California residents)
- Fill in the quantity of each item in your order, in the yellow cells. Shipping will automatically be added. If you’re a California resident, sales tax will also be calculated and added.
- Fill in your personal information
- Save and print the spreadsheet
- Write a check for the total
- Mail the form and check to:
